Card Validation & Registration Form

You will need to register your card before its first use. Once registered, only that person may use the card.

You will find a grey scratch off bar on the back of the card. Once scratched off, there are a series of letters/numbers on the back of the card that you will need to enter into the web form in order to register the card.

The format is as follows:
